Most people need some form of long-term care support, housing or services during their lifetime. Physical and cognitive impairment, as well as general aging, will make this type of care necessary. You may wonder, “What percentage of Americans will need long-term care?” Although the level and intensity of long-term care needed varies by individual, the Administration on Aging estimates that at least 70% of people age 65 today will need care in some context. Long-term care facilities include nursing homes, assisted living communities, and skilled nursing facilities. Each of these types of care offers unique facilities and services.
Skilled nursing facilities (SNFs) provide the highest level of medical care for any long-term care facility. This care is performed by registered nurses under the supervision of a physician. This is similar to the level of nursing care a patient receives in a hospital, but in a more comfortable, residential setting. SNFs often also offer rehabilitation services, physical and occupational therapy, and post-surgical care.
SNFs aim to provide transitional care — in other words, they are designed for short- and medium-length stays of several months rather than the resident’s lifetime. In some situations, a short stay in a SNF may be covered by Medicare.
Nursing homes also provide around-the-clock medical care and are typically supervised by licensed practical nurses and nurse assistants under the supervision of a limited staff of physicians or registered nurses. Nursing homes provide assistance with medication management, wound care, and medical needs, focusing primarily on assistance with residential, custodial care, and activities of daily living (ADLs).

Some nursing home residents stay only for short-term rehabilitation, while others stay there for the rest of their lives.
Assisted living communities, unlike nursing homes and skilled nursing facilities, are not designed to provide comprehensive medical care. Assisted living residents are generally active but require assistance with some ADLs such as dressing and bathing. They enjoy a supervision-free lifestyle where meals, activities and housing services are provided, and they usually live in private rooms or apartments with access to community facilities. Some assisted living communities may have memory care wings or secure memory care units for those with memory loss challenges.
Home health care agencies can also provide long-term care. This care is provided in the senior’s home and includes both ADL assistance and medical care depending on the patient’s needs.
Doctors prescribe hospice care when a patient is expected to live six months or less from admission. Some agencies that provide data sets, such as the National Center for Health Statistics (NCHS), include hospice care under the umbrella of long-term care, while others, such as the Centers for Disease Control and Prevention (CDC), do not.

Making The Nursing Home Choice
Many residents in the 65-84 population live in assisted living communities, while older residents live in nursing homes or SNFs.

While many equate long-term care with an aging population, residents between the ages of 31 and 64 have been the fastest-growing nursing home demographic over the past 10 years. Although this age group still represents only 7% of long-term care residents, their impact on the state of long-term care facilities is noteworthy.
These “young adults,” defined by the Society for Post-Acute and Long-Term Care Medicine as people between the ages of 18 and 64, typically face different health issues than the over-65 population.

This demographic often has different care needs than the seniors with whom they live, and long-term care facilities may have unique activities and amenities designed for younger residents.
Because long-term care facilities include a wide range of care types, different residents have different needs. Seniors in assisted living communities may only need assistance with some activities of daily living, while older adults in skilled nursing facilities may require intensive medical care.
Sixty-one percent of long-term care residents require assistance with three or more activities of daily living (ADLs), such as bathing, dressing, using the toilet, or personal grooming.
Assisted living residents are less likely to need assistance with multiple ADLs and are more independent. Among nursing home and SNF residents alone, more than 80% required assistance with three or more ADLs, and more than 90% of those able to walk required supervision or assistance.
Nursing Home Care Vs. Home Health Care
Recent statistics regarding ADL assistance in nursing homes and skilled nursing facilities are not publicly available, with the last robust dataset reported in 2014. However, assisted living ADL assistance is tracked by the American Health Care Association.
In 2020, an estimated $3.4 trillion was spent on personal health care resources in the US, of which about 14%, or $475.1 billion, was spent on long-term care facilities and services.
Long-term care services and facilities are funded by both public and private sources. Public and government agencies like Medicaid and Medicare accounted for 60.4% of spending nationwide in 2020, according to the Congressional Research Service.
In 2020 and 2021, Pandemic Assistance contributed to long-term care and services funding, which accounts for about 6.3% of spending. Pandemic funding is unlikely to continue for long.
Medicaid’s Role In Nursing Home Care
Private sources accounted for 27.7% of long-term care spending in 2020.
